Have you recently received a booster dose of a COVID-19 vaccine (Pfizer, Moderna or Johnson & Johnson's Janssen) either through Baylor Health Services or an off-campus provider?

If you have received a booster, please upload your updated vaccination card (image or PDF) that includes the booster dose by using the Confidential Employee Medical Record Form on the University's coronavirus website. (To successfully access the form, you must be logged in to your Baylor-provided email account.)

The form is short and requires your name, University ID number and your affiliation (faculty, staff, student employee or student (non-employee). On step 5, select "I have been fully vaccinated" and click Next. Follow the instructions to upload your completed card.

The booster information will continue to assist the University in understanding how well protected we are as a campus community as we monitor the latest variant.

The same Confidential Employee Medical Record Form can be used for students or employees who have recently completed their full vaccination to upload their vaccination card.

Baylor does not require a COVID-19 vaccination or booster. At this time, you are considered fully vaccinated two weeks (14 days) after the second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or two weeks (14 days) after a single-dose vaccine, such as Johnson & Johnson's Janssen vaccine. As of Jan. 18, 2022, more than 80% of the campus community is fully vaccinated.
